A short, clear description helps the support team understand the problem and get it to the right person. Write down the details while they are fresh, and always use your approved support and data-handling process.
Describe the environment
- The application, workflow, device type, and location involved
- The user role or affected group without unnecessary personal details
- Whether the issue affects one user, several users, or an entire workflow
Tell us what happened
Write the shortest sequence that reproduces the issue. Include what you expected, what actually occurred, and whether retrying or changing environments affected the result.
Record timing and impact
Include the approximate time, frequency, operational impact, and whether work can continue through an approved alternative process. This helps distinguish an isolated question from a time-sensitive interruption.
Include evidence carefully
Record error text, identifiers, screenshots, or logs only when your organization permits it and only through an approved support channel.
Protect sensitive information
Do not include patient information, credentials, access tokens, or other sensitive data in an ordinary email or web form. Follow your organization’s privacy, security, and support procedures.
